Liposculpture / liposucion

During a liposuction fat is sucked from under the skin via small incisions of half a centimetre. The surgeon only removes the fatty tissue and does not stretch the skin. It is therefore important not to have too loose skin for optimal results.
 
Liposculpture is a relatively new concept. It is similar to liposuction but is done by means of finer cannula and following a more refined technique. Only one or two areas can be treated per procedure under local anaesthetic. If larger liposuction is required, the surgeon then operates under general anaesthetic. Mediclinic has the safest anaesthetic methods allowing the surgeon to remove larger quantities of fat per procedure.
 
There are various techniques for liposculpture. The simplest is that in which the cannulas are connected to the hand operated suction pipe. This method is more intense for the surgeon but still gives a fairly wide control. Vibroliposculpture is currently very popular, and is done with an automatically vibrating cannula. The surgeon therefore uses less power and can work faster. This method is also easier under local anaesthetic owing to less sudden movements.
 
Ultrasonic Liposculpture
During an ultrasonic liposuction the cannula is connected to an ultrasound head. This energy transforms fat into a white liquid. The advantage being that liposculpture can be performed even in dense tissue. The device also has a second advantage: it shrinks the skin which becomes firmer, and stimulates the production of connective tissue underneath. Ultrasonic Liposculpture is the most effective solution for tightening the skin without scarring. Even mini-breast lifting can be performed without scarring.

Liposuction is probably the most underestimated surgery. The incisions are very limited, but the working area is much larger. This surgery is very safe, provided of course, that it is conducted in sterile conditions and that the amounts of fluids administered and the fatty tissue sucked off remain within reasonable limits.
 
Our body contains different kinds of fatty tissue. Each type has specific properties:

  • The fatty tissue on the hips and thighs is very sensitive to oestrogen and somewhat insensitive to dieting. The great thing is that once removed, fatty tissue never returns.
  • The fatty tissue on the abdomen is a storage tissue. This is a very popular area for liposculpture. Results are very good and are can be seen immediately in clothes, but they will be less noticeable on the thighs if the patient puts on a lot of weight afterwards. The reserve areas respond well to dieting. Reserve fat is also located inside the thighs.
  • Finally, there is the structural fatty tissue that lies deeper between the two above mentioned tissues and almost everywhere under our skin. Structural fat also reacts well but much less than the reserve fat. The structural fat has a firmer texture and must be removed in a more careful and very homogeneous manner. The most popular areas for treatment are the "love handles" which are located on the sides at waist level. Structural fat is also found on the back, shoulder blades and armpits. Because the fatty tissue is firmer the results of ultrasonic liposuction is most attractive.

Since July 2003, the Belgium states that only dermatologists and Surgeons (plastic) can perform liposuctions. Dermatologists are limited to a litre per session. General practitioners, aesthetic physicians, dentists, ... can no longer perform liposuctions in Belgium.
 
In practice:

Pain: The treated areas are bruised and tender to the touch during the first days.
Convalescence: Generally there is some discomfort with small liposculpture, but when more than 2.5 litres are drawn in one session, fatigue and low blood pressure may occur for one to two days. Treated areas remain inflated for a week. The final result is obtained after 6 weeks. The belly deflates more slowly. Too rapid return to work or intense sport slows the disappearance of swelling.
Risks: A liposculpture of less than one litre carries little risk. The scars or infections rarely pose a problem. Bruises appear but disappear after a week to ten days by themselves. Seroma may appear on the abdomen as a result of extensive liposculpture. These small accumulations of liquid disappear by themselves after a few weeks. During liposculpture of over 3 litres, the risks of pulmonary embolism, fat embolism and underfill increase. That is why we limit the amount of fat extraction to between 2.5 and 3 litres.
Postoperative care: After liposculpture, a compression bandage should be worn for two to six weeks. Directly after the operation, you will be asked to drink more water or lightly salted broth. For 24 hours, liquid may seep from small incisions. This is normal and disappears spontaneously.
 
Liposuction is one of the most popular procedures. If some basic rules (hygiene, maximum amounts, etc.) are met and if the surgical technique is correct, the operation is very safe and leads to patient satisfaction. Given the recent law by which only plastic surgeons and dermatologists may practice liposculpture, the quality of interventions is increasing and the number of complications decreasing.
